I know this is moot now, but I wonder in such tall buildings: How did they take smoke breaks in the WTC Towers? Were there well-vented rooms reserved just for smoking on every other floor, maybe?

No, they had to go all the way to the ground floor and out onto the plaza to smoke. I’ve heard interviews with people who survived the attacks for the sole reason that they are smokers.
