First clarify whether or not you’re talking about porn.
Then tell us how many times you’ve had sex with a movie crew watching.
Most interviews I’ve heard with actors who have been in nude scenes say that the circumstances are usually not conducive to becoming aroused. You’re surrounded by people and hot lights, people are working, there are frequent stoppages, there’s the pressure and concentration on producing a good performance, peopel kep giving you directions, etc.
I’m sure an erection happens on rare occasions, but that kind of thing doesn’t have to result in actual genital penetration. There’s a lot of starting and stopping in movies. Most directors aren’t going to force an actor to continue through an erection. There are rumours occasionally that real sex has taken place. Specifically, I’m thinking of the sex scenes between Mickey Rourke and Lisa Bonet in Angel Heart.
I remember seeing one of those “blooper” shows a long time ago that had a takeout clip from a British soap opera. There were a man and woman in bed together getting hot and heavy and suddenly the man wrapped the sheet around his waist and jumped up, with many apologies. In other words, he stopped the scene as soon as he got an erection.
They’re also not usually naked, either. Most scenes, one or both partners are wearing something, and they just choose camera angles so the undies don’t show. Or edit them out in post-production.
I mean, really, how naked do you have to be for the typical under-the-sheets scene?
And even if they appear mostly naked the actors are often wearing nearly invisible crotch covers. In most shots they can actually wear some kinds of underwear. Unless you’re watching the rare movie with inconcealable full-frontal nudity, the actors are almost never completely unclothed.
The sex scene in Ang Lee’s Lust, Caution (2007) was reportedely real, and I can believe it. From the Trivia section in that link: “The rather explicit sex scenes were shot over 11 days on a closed set, with only the main camera and sound personnel present.”
The scene in Pola X (1999) looked pretty real. That sure looked like his penis going into her vagina.
It’s reported that the oral sex scene between Vincent Gallo and Chloe Sevigny in Brown Bunny was not simulated.
The film Short Bus is supposed to have all unsimulated sex scenes as well.
I forgot The Brown Bunny. That was extremely explicit. Chloe Sevigny herself has stated it was real. I believe they had dated in the past.
9 Songs was also real, although that was less a film and more a compilation of bad porn and bad indie bands.
Sweet Sweetback’s Badaaaaasssss Song (or whatnot) is fairly famous for having unsimulated sex scenes, despite (to my knowledge) not explicitly showing anything that couldn’t have been simulated.
The lead actor / director just really wanted the “authenticity.”
In the unlikely event that the couple was actually naked and the actress/director didn’t have any objections and they kept filming while he was erect there wouldn’t have to be any penetration at all. He could slide it against her vulva, pubis mons, belly, or between her thighs, depending on the position and angle of the shot.
Even in porn there are a lot of ‘cuddle shots’ where no actual penetration occurs.

I remember once watvhing some sort of documentary (on HBO or something) about makle porn actors and how hard it was to get “good ones”.
