Hey Dopers, I need to submit a small bibliography and I’ve lost my notes on proper format.

Is that how it should look? I’ve never really done any bibliographical work before (first major paper I’ve written) and I just wanted to make sure things were listed right etc…
Before anyone can provide any substantial help to you in this matter, you’d need to identify what STYLE of citations your professor/teacher has asked you to use. The main three are:[ul]
[li] Modern Language Association…frequently used in literature, the arts and the rest of the humanities[/li][li] Chicago/Turabian…many “real-world” non-scholarly publications[/li][li] American Psychological Association…frequently used in education and behavioral and social sciences[/ul][/li]
Once you’ve got the style you need figured out, the link will provide you with the proper format for your bibliography.
Let me put in a suggestion to look into getting EndNote, a wonderful program that creates bibliographies from the info you input – in a number of formats. You can download a free 30-day trial version from their website here. That’ll take care of your problem for now, but it’s definitely going to be worth your while to learn at least one format by heart.
