Actually, I had Dr. Death in mind for the first answer. Harvey Dent suffered disfigurement from having acid thrown in his face, not from a chemical explosion as far as I can tell. Dr. Destiny is correct for #2, and Doctor Double X is the answer to #3. Doctor x 3!
