Right, it seems like this was in answer to one question, not a formal greeting. I see nothing which sez GWB did not formally adress the Pope as “Your Holiness” when they first met.

I wouldn’t call her “Your Highness,” either.
I would, however, address her as “Your Majesty.”

(If I recall correctly, you’re supposed to address the Queen at first, with “YM”, and then, when you refer to her next, just “Ma’am.”)
