Hmm, I think I may have inadvertently been misleading - the last part is probably better phrased as “things are looking up, and down!”. I’ll give it another day or so and if no-one gets it, will move on.

Correct! Though I must admit that for some while, I had confused endoscopy with colonoscopy (just as well I’m not a doctor), hence “looking up”. I think the instrument used to perform a colonoscopy is still a type of endoscope, though, so it still works. But my original clue simply didn’t have enough information. The revised version had an anagram (indicated by “puzzled”) - it was “doctor”, without “RT” (retweet), plus “pen” and “ses” (“mostly sees”). I thought the ‘doctor’ and ‘pen’ (an endoscope could be described as a ‘camera pen’) elements might also lead people towards the answer.
