What were they?
According to http://www.dtic.mil/whs/directives/infomgt/forms/forminfo/forminfopage1508.html DD form 1 is for Officers commissions.
And according to this site http://77fa.org/buddy.html DA form 1 is the daily unit roster (scroll down to Morning Reports).
This is all I’ve been able to find on these forms so far. There is no information (that I found on these pages) about previous versions so they may have changed over time.
