A space station toilet seat fell on her head.
She runs into people who knew her fairly frequently (in fact, I’m pretty sure she’s working where she worked before she died in her day job), but she doesn’t look like herself any more.
Great show, and sorry it was cancelled!
I believe it was a toilet(?) or the frozen remains from a toilet, that fell from a plane and landed on her head.
She was dead, but her body had a new form, so at one point you saw her “new” human form which was much older and world-weary (to put it nicely) and thus, unrecognizable to her friends and family.

I loved Rube’s relationship with gah, I can’t remember the english guy’s name. As soon as I do, I’m going to hit myself. Starts with an “M” I think. Anywho

Answered above, but you should really pick up the DVDs ( rent or buy ) as the pilot answers these sorts of questions. And I’m not being snarky - I just think you’ll enjoy them more watching them in order :). There is a progression of sorts and ongoing plot archs ( with her family in particular ). That said, they also start to play a little fast and loose with some of the ground rules laid down in the first episode, but that’s showbiz for ya and it doesn’t seriously compromise the quality.
There are two seasons ( all that were made ) available and if you like Dead Like Me, I’d also strongly suggest you give the lamentably short run of Wonderfalls a try. It’s from the same production team and is very similar in tone ( snarky ) and quality.

Just to be more specific, They mentioned the Toilet seat was a stainless steel Toilet seat from Mir.
The DVD’s are availalbe for Netflix. That is how I got to watch the entire series.
