What diet and exercise system is considered to have the fastest results in terms of Lean Body Mass to Body Fat % ratio positive rate of change without steroids, surgery or supplements? I know this is an odd question to ask, but I’m looking for the practical limit not advice, and I realize that a lot of this(if not all) is highly individual-specific.

It is a common mantra among dieticians, body builders and even laymen that your body is in either catabolic or anabolic state. Specifically that if you are burning fat tissues for energy because of a caloric deficit, your body will not build new muscle at the same time, and that you need a caloric surplus to build both fat and muscle. I mean on biological level it makes sense to me, but a interesting question comes to mind. Say a person is in caloric deficit and is burning adipose tissue for fuel, then they suddenly start heavily lifting weights, tearing their muscle fibers. Now under normal circumstances the body would repair the muscle fibers creating more , stronger muscle. Are these people say that if you are in a catabolic state, that your body will just not repair torn muscle fibers? That seems against everything I know of human physiology - i.e. if something is broken your body will try the best to fix it at all costs. So if the body does repair those torn fibers, why is it considered impossible to agressively burn fat and gain muscle at the same time (consistently)?
