How do you handle a child that is a very picky eater?

The larger picture is child-rearing. This must be done with firmness and discipline, which on the other hand needs to be characterized by trust and faintness. A child will respect parental rules, as long as the child can see that he can trust the parents to administer fair rules. The parent has the responsibility to see that rules are fair, and to see that expectation of trust is a two-way street.
