I’ve heard that two stuntmen died in the Aston Martin chase at the beginning of Quantum of Solace (although a cursory reading of the Wiki page shows the accident was not actually lethal). I’ve also heard that a Bond girl died during the filming of Goldfinger (but Wiki doesn’t confirm that one).
