I’m on the fence. Could go either way depending on the circumstances.
My children are cuddlers. They love being cuddly with me and with each other. But there are limits. Sitting between thighs isn’t a sibling cuddle position, so I’d have to go with, “eh, that’s creepy.” But like the photo, not so much. In our house, though, the genders would be switched. My son would be leaning against my daughter since he is quite a bit younger than she.
Not creepy as long it stays short of this.
That was hilarious! It also made realize that’s it’s been so long since I watched SNL, I wasn’t getting why the audience was cheering when certain cast members walked on stage.
It’s creepy. That doesn’t mean anything creepy is going on, but it looks like there is, so it’s still creepy.
My barometer: If I walk into a room and see two people acting/sitting in a manner that would make me think they’re either dating, married, single and flirting or hoping to be “more than friends,” then I would hope they’re not brother and sister.
If I saw two adults (young or otherwise) sitting the way the OP described, I would think the two were some degree of an item. So, to me, it’s inappropriate (unless, they’re an item and everyone’s ok with that).
Would I be squicked out if my wife was sitting that way with one of my male friends? Yep. And the same thing goes for one of her brothers. But, of course I realize, YMMV.

5 sisters, all on the porch sleeping in a row of single cots. Also me & my brother. Once in a while Grandma would be out there too.
Doing water things, surf boards, tubes, building pyramids with skiers, cliff climbing, all kinds of touching going on with a lot of different people.
As to a really sexual position that would make Grandma get upset, would need to know more about the people and circumstances.
Just strangers I had never met before, just sitting, does not bother me. Now, if between 40 & 60 year old’s when I find out they are siblings, I may roll my eyes.
Depends on the people. The sight alone wouldn’t creep me out unless I question the actual relationship.
As long as any actual incesticating is mutually voluntary, I don’t care what they do.

But, snuggling for a long time is right out, and creepy IMHO.
I hug all my friends, and have a great picture of my Wife and her Brother hugging and laughing on the couch. Shows love for siblings, and nothing else.
It’s different everywhere. I understand that male friends in Arabic countries will hold hands just walking down the street.

I’m a prude because it would weird me out to see siblings act like the OP’s pic.
Doesn’t creep me out at all. I wouldn’t do it with either of my sisters, we’re just not that close, but I have (precisely the position described in the OP) with a female cousin, us being in our 30s. As long as no-one’s hands are on boobs or butts, it’s just cuddling.
I voted creepy but would like to know where the guy’s hands were before really being creeped out.
