It’s Lena Dunham, so by definition it will be pretentious and probably a little racist.
An unreserved apology would not have included references to her own insecurities. It would have been an unreserved apology.
(I don’t know what the hell her weight has to do with this topic, and would point out that a woman of her height, were she to lose 50 pounds, would be left with the physique of Jack Skellington.)

It is possible Dunham did not have a racist intent, but the effect of her comments had a racist sound to them no reasonable person can deny, and which she admits to be the case. Whether it was her intent or not, it sounded racist.

As to whether that was her intent, it’s probably best in interpreting her comments to simply assume that they are consistent with her performance art shtick, which is basically her branding of herself. Lena Dunham’s thing is to sell Lena Dunham. She has a show in which she plays Lena Dunham under a different name, made a movie in which she stars as Lena Dunham under a different name, has played a variety of roles in which she is Lena Dunham, and wrote a book about being Lena Dunham. It’s what she does for a living, and as it made her quite rich she is, quite logically, making hay while the sun shines.
The theory that she does this because she’s a rich kid who has known nothing but being spoiled is a plausible one, but I’ve never met her.
A core characteristic of narcissists is that they dish out rejection to others with glee and schadenfreude, yet froth with rage when themselves rejected.
