Is there a program that creates documents where stock questions can be answered...

Damn that Title character limit!

basically, what I want to do is create a document based on stock questions.

I work with the student Senate where legislation is drafted. the legislation is always the same format . However, everyone seems to change the format.

When it appears on an agenda, even the smallest changes are noticeable. so is there a program where questions are asked, data is provided, then the data is applied in layout that has been already established?

such as:

“What is the type of bill?”
A: Bill to Fund
B: Bill to Recognize
C: Bill to change guidelines

(or an input box…)
Who is the recognition affecting?_________

Who is the bill written by?________

then, when done, the document looks cohesive and consistent… the information given would go into a layout and look something like:

Bill to Recognize Investment Society for Humans
Written and Submitted by: Kenneth Lay

this is just an example but there will be about seven questions asked and it will fill up one-page. I have Microsoft Office but I have not seen (or found) where this may be accessed.
any ideas? I want to make this as easy as possible.

You can do this by creating a template in Word and employing macros. I haven’t done it recently enough to remember the exact steps, but it is definitely possible. Look in the help file for information on ASK and FILLIN fields.
