John Lackey is pretty well known for not wanting to be pulled from games but I don’t know of any tantrums. I remember in the '13 World Series he successfully talked John Farrell into not pulling him.
One of the Indian’s pitchers punched something (paper towel dispenser?) a few years ago and was out for quite a while. Because he broke his hand. (No video just article)
Not pulled by the manager, here’s one of Mark Lemongello (one of the best names in baseball history) throwing a tantrum after being tossed by the umpire.
Not a pitcher but there was an incident on national television June 17, 1977 when Yankee manager Billy Martin removed outfielder Reggie Jackson for not hustling fielding a baseball. Both had a strong dislike for each other and were not shrinking violets about it. Words were exchanged and ultimately coaches Elston Howard and Yogi Berra had to physically restrain them. Carlos Zambrano was one of the famous incidents. There was some Gatorade punching during that Cubs - Pirates wild card game, but I already forgot who it was doing the punching, and I don’t think it was a pitcher anyway.
All started by a horribly blown call in the 9th inning the game before. There shouldn’t have been a game 7. Which is what Whitey Herzog is basically saying when he gets thrown out.
Can’t link to it from work, but Kershaw was more than a little miffed to be pulled a couple of starts ago. He and Mattingly had words in the dugout. A whole lotta words.
Oh, actually, I was mistaken about Zambrano a little bit. He went to town on the Gatorade with a baseball bat. ETA: Memory is a funny thing. My recollection of the event was punching a cylindrically container of Gatorade, but that might have been influenced by seeing that Rodriguez footage a week or so ago. (Unless Zambrano had another Gatorade incident.)
The Blue Jays’ John Gibbons/Ted Lilly altercation of 2006. It started on the mound and resumed in the tunnel to the dugout - Lilly denied that any punches were thrown but something heated sure happened at the 47-second mark of the clip.
Sure thing. Cards shoulda won that series. But deciding to argue balls and strikes in the next game, and particularly choosing two pitches that were clearly outside, seems like an odd hill to have planted his flag on.
Just one symptom of the Cards complete collapse after being deflated by having the previous game stolen from them. By the time Andujar came in it was already a rout.

When informed he was being sent down to Syracuse, he threw an ashtray at the head of Blue Jays GM Peter Bavasi, barely missing him.
In 1982, a few years after leaving baseball, Lemongello and Manuel Seoane (a former Wichita teammate), were arrested for the kidnapping and robbery of Lemongello’s cousins Mike Lemongello, a former professional bowler, and Peter Lemongello.
Definitely blown call which should have been first out instead on man on first. But the Cards self destructed after that…passed ball moves runners to 3B and 2B with one out, Herzog intentionally walks McRae to load bases so Royals get platoon advantage with Iorg where Worrell has to throw strikes. Iorg gets game winning hit. Let’s just say I don’t like loading the bases especially less than two outs.
