Can’t expect people to follow our rules all the time. Just take it how they give it and let it go. They will get it wrong in some way no matter how you ask. And go for the McDouble. I can’t abide by McD food, but I will go to Wendy’s for a triple and a side salad and lose a pound that day easy.
It sounds to me like a communication problem. At the McDonald’s I work at we have customers which regularly order similar items. We even have a key for No Bun. It comes down to how the crew people are trained. I agree that coming inside usually allows for better service. If you are a “Regular” the crew should get to know your order. Some crew members will actually start assembling orders when they see certain customers.
You’re not a psycho bitch for wanting your food specially prepared. Trust me I’ve dealt with many psycho bitches. You’re not even an annoyance.
They have a “No bun” key which isn’t the problem, but they don’t have a “put two sandwiches in one box” key.
The McDouble (or double cheeseburger) without the bun would solve your problem and be cheaper. For breakfast, instead of ordering two Sausage McMuffins, order one Sausage McMuffin with an extra sausage patty (and extra cheese if you really need it).
Do whatever works for you. When in tried Atkins, I found McDs to have a lot of great options (you can order scrambled eggs and bacon in the AM.)
How long have you been in Atkins this time around?
In the OP you describe the plan as “all meat, no carbs.” That’s pretty far from the truth of how Atkins works so I hope that’s not actually what you’re doing. You should be getting 15 - 20 carbs per day, mostly from vegetables. Are you?
I agree that it might help to go inside, and also wear an unusual hat or something every time, to make yourself memorable. Then each time they package it wrong, politely mention that you’re trying to help the environment so tomorrow you’d appreciate it if they could pack it in one container like you asked.
If you think they have too much packaging, you haven’t been to KFC lately.
When it comes to fast food (and I get as annoyed as anyone with messed up orders), I can still hear the words of a former co-worker echoing in my ears 15 years later: “When it comes to people being paid $7 an hour to give you food, how much do you think they really care what you end up with?”
You have been misremembering then. I worked fast food 15 years ago and I can tell you no one made $7 an hour. It was something like $4. But the sentiment remains, they don’t get paid enough to care, then or now.

Another low-carb option is the Whopper “platter” at Burger King. It’s a Whopper in a salad bowl minus the bun and ketchup but with more lettuce. A few years ago, we had a stab at the South Beach plan and my experience was that some BK people knew exactly what the platter was and others had no idea at all, but asking for no ketchup, no bun, extra lettuce generally got things going.
