Turns out TV Tropes has a page on nameless characters, although many of their examples are cases where the name was eventually revealed or where the character has a first or last name but not both.

When he receives the telegram informing him that he’s been called back, you can read the name and address upside down after he hands it to Withnail.

Huh. I’ve watched that film a dozen times and never noticed. Thanks! Does it show his first name too? What is it?

The guy who shot House at the end of season 2 is never referred to by name in the show… considering he is actually there interacting with anybody at PPTH during the cold open. He got out of the hospital before security or the police could grab him.
I believe he was credited as Moriarty, in reference to Sherlock Holmes’ nemesis, but never called that by name.

I hope I did that spoiler right… I’ve never done one before.

For all of Kill Bill Pt 1 and a good chunk of Pt 2, Uma Thurman’s character is known simply as “The Bride”.

I can’t recall her ever being referred to as that in the movie. I can only ever recall her being called “Black Mamba”, regardless of what she may be credited as.

Tarantino is very cute on this point - you eventually discover you’d known her last name for a while:

Bill calls her “kiddo” on several occasions - we eventually learn her name is actually Beatrix Kiddo.

And what - no love for this guy? I know he eventually revealed his name (or did he?) but he spent a full eight seasons as The Janitor.

Being called something and being known as something are different. Uma’s character is referred to as “The Bride” with on-screen text. I believe she’s introduced in the opening/closing credits as “Uma Thurman as The Bride/The Bride - Uma Thurman”.
