Programs for the Hard Core Homeless - Philadelphia and Elsewhere

My limited knowledge of programs for the homeless (this is in Philadelphia) mostly pertains to programs to attempt to get homeless people to “reform” in one way or another and seem to revolve around housing those who can follow the rules. What are some successful programs that care for the homeless without requiring any behavioral change? I know when it gets cold enough in Philly there is a city agency that attempts to get everyone inside for the night. What about during the rest of the year? There has to be some foundation that recognizes that some people will never change and that this rigidness is not a justification for letting them suffer. I’d be interested in hearing about Philadelphia or anywhere else in the US.
