Question about in-text citations...

So I’m writing a big paper today for school, and I’m having trouble with the in-text citations. I know that if you cite the same author many times in a row, you should just use the page number instead of putting in the author’s name over and over with it. But:

  1. Should the first of the citations have the author’s name and page number, and then the citations following it will only have the page number? Or

  2. Should all of the citations have the page number only, and then the last one in the series should include the author’s name?

  3. How many citations from the same author should there be in a row before you start only putting in the page number?

  4. When you do have an in-text citation with only the page number, does the citation still go before the period?

For example, would it be: Freud believed that the dream wish originated in the unconscious (78).
Or would it be: Freud believed that the dream wish originated in the unconscious. (78)

If this is somewhere in the MLA handbook, can someone please tell me where it is? I can’t find it anywhere.
