I added “non-porn” because I don’t want to turn this into as thread full of "This one hot chick goes down on these two guys, and then gets a cream pie, and then . . . " posts.
Rather, looking for movies that were at least some sort of attempt to be released in a movie theater that did NOT encourage Pee Wee Herman jacking off in the third row.
My nominees are:
“Cannibal Holocaust”- Among the outrages: actual killing of a live turtle and coati; 2 rape scenes; one vaginal mutilation; the killing of a newborn baby and its mother; cannibalism, of course; and that’s just the worst stuff!
“Bloodsucking Freaks”- Woman has her teeth pulled out, one by one without painkiller, and then the villain cuts off the top of her skull and drinks blood out of her brain; another has her arm cut off; and in yet another scene, a schoolgirl has the release rope to a guillotine put in her mouth and is spanked until she yells out, beheading herself
“Caligula”- OK, borderline porn, but I don’t believe it was originally shot to be a porn film. The worst scene in this one is a Roman soldier has his dick tied off, they fill him up with wine, and then cut his bladder. He is then castrated, and his private parts are thrown to dogs. To cap it off, two women pee on his dead or dying corpse. Not the way I’d want to go!
Those are the three that come to mind. Some others include:
“Pink Flamingos” (we ALL know what scene)
“Vulgar”
I’ve also heard “Salo: 60 Days of Sodom” is pretty twisted as well, but have not been able to get a copy.
Last House On Dead End Street is one of the few movies to give Bloodsucking Freaks a run for its money. Like BSF, the version that is available is, as extreme as it is, a greatly cut down version of what was originally intended. Both movies also have in common that they are ‘about’ the creation of ‘extreme works’ that people confuse for ‘fake’. They’re also both brilliant works of art in their own right.
Salo is a strange movie, true to its source in De Sade’s ‘120 Days of Sodom’, while also being a denunciation of fascism, and seemingly wildly homophobic (even though Pasolini was himself gay). It was on Youtube a while ago (and may still be up), but without subtitles.
I’d argue that Pretty Woman is sicker than any of these films, though.

Haven’t watched it myself, but I hear I Spit On Your Grave is pretty soul-destroying. It’s about a woman who’s gang-raped by five men and gets bloody revenge on each of them.
I’d suggest Guinea Pig: Flower of Flesh and Blood, a somewhat infamous fake snuff film. Basically, serial killer in Samurai armor dissects a young lady on-camera over a couple of hours. Actually has a pretty catchy a cappela number towards the end. Realistic enough that Charlie Sheen tipped off the FBI after seeing this, thinking it was real (cite). Available on Netflix, last time I checked. Apparently was once aired on San Francisco public access television in 1996, somehow.
I’d also suggest Nekromantik 2, which is just about what you’d expect, and just as awful. On the other hand, it’s also boring. So boring it’s actually relaxing—I’m not kidding, the damn thing actually put me to sleep when I tried to watch it. The fact that it has a violin score that makes Phillip Glass sound like Hendrix probably had something to do with it.

There’s that french movie with the seemingly endless rape scene - Bais Moi. Tough stuff
August Underground’s Mordum was the sickest movie I’ve ever seen, but I think it was direct to video, not a theatrical release. It was so disturbing it almost made me vomit.
Peter Jackson’s Dead Alive aka Braindead claims to have the largest blood flow of any movie, and is extremely gross. Jackson has a reference to it in his King Kong.
The Devils/The Devils of Loudoun was probably the sickest movie I saw that wasn’t actively trying to be a gross-out film.

I am NOT into these types of movies at all. I hate gore/sick stuff. But I did try watching the short mentioned in the list I linked, Cutting Moments. As soon as I saw one of the gross parts just begin to happen I had to turn it off. I am not cut out for this type of extreme, straight-to-video, underground copy cause it’s banned type stuff. So I can’t comment on if these are really sick or not but I do remember a particular thread on Fark and a LOT of the people were mentioning the movies in my link, so I guess it’s a decent list.
I’ve seen this (or at least large parts of it, I have tried to put it out of my mind). The mildest revenge is where she chases down one or two of the rapists in a lake, with her in a motor boat, and chops them up with the rotor.
There is one scene in there that no man should ever watch. It does for bathtubs what Psycho did for showers. Sometimes late at night I flash back on that scene, and cringe doesn’t begin to cover it.

I Spit on Your Grave is a great film. What makes it hard for some people to watch is that it’s a woman exacting violent revenge on a bunch of male rapists. This goes so counter to what we expect from a movie–the woman is supposed to be the almost willing victim, dutifully and attractively put in her place by the powerful man, then she should sit quietly sobbing and drinking herbal tea while another powerful man goes out and avenges her–that it enrages some people. It’s a pretty strong feminist statement, which is not what people go to slasher films for.
