I saw it as she’s about to stomp on his foot with her high heels. Which is a thing I taught women to do many years ago, when I taught self defense in university.
I think that this style of expression is intended to be ambiguous, that we can interpret it as it strikes us, and that it is not necessary to decide what it is “supposed to be.”

I take the green bits as showing motion. Perhaps the movement of her skirt.
Maybe the artist had problems with women, himself.

If he had an intent. It may be pure emotion that guided him.
