Which do you prefer?
I hate to be the one that says “you forgot…”, but I always spell it “Ok” which is not an option.
They are all actually OK with me, but I usually type OK.
I use OK myself. O.K. seems a bit stuffy/archaic. “ok” and “okay” both seem OK, though informal. I would only use those when instant messaging or texting or sending an informal email.
“OK” and “ok” (and Ok) are pronounced “ock.” (At least in my world.)
“O.K.” and “okay” are pronounced “okay.”
Being too lazy to type punctuation, or two extra letters, is a special, weird type of laziness.
I don’t want people to get confused with Oklahoma. I type “okay” usually.
Unless I’m talkin’ about Martin Van Buren, I either use OK or okay, mmmkay?

I always just type ok, because that’s the word now. Okay is a phonetic variant that I don’t mind seeing but don’t use myself (unless I’m voice texting, then it puts it in automatically). OK is unnecessary. O.K. is right out!
OK, Ok, or ok for rendering speech. Okay or okay for noun and verb forms.
“Okay”, normally. “OK” or just “k” if I’m trying to come off in text that I’m annoyed with the person I’m talking to.
Any way except O.K. because punctuation is a PITA when texting.

My preference is “okay.” Once a locution becomes an independent, pronounceable word, it should no longer be treated as an abbreviation or initialization.
The OP also forgot an archaic form, “okeh.”

The majority of the time I spell it out, “okay.” I will often stop myself and wonder if that’s right, considering its origin is supposedly “Oll Korrekt” which is a story I choose to not accept because it is wholly unsatisfying. So then I continue to spell it “okay.”
OK, because it’s not an abbreviation… (checks the Online Etymology Dictionary, one of his favorite websites ever) …except holy shit it is. Still, O.K. is a K.G. for me.
