The 11th Hour. That game was just shitty compared to 7th Guest … and yeah, I know what I’m saying there.
That’d be Ultima 5, m’boy, and you’re talking of the sandalwood box that contains the Orb of the Moons which gets you out of the Underworld. And yeah, the first time I played that I completely forgot to pick up that box, and had to restart and reply, as I only had the one saved game.
After you beat Diablo (or Baal if you have the expansion), you will have the choice of sending your character to the next level of difficulty. Do so, save the game, resume game at the previous level, then use the Horadric Cube to transmute a Tome of Town Portal and Wirt’s Leg to open a portal to the cows. Moo!
By the way, as long as the Cow King remains alive your character can return later to kill more cows. This may be changed in v. 1.10 though.

Well wouldn’t you freakin’ know it. I thought I was in the clear until the last post was the answer I was hoping to give. Ah well.
Here are some purdy obvious ones:
