Trying to find a madrigal song ... and, obviously, failing

This is my first Cafe Society thread or post (at least, that I can remember). Be gentle:)

When I was in high school, the madrigal group I was in did some old-ish (pre-1907:D) SATB madrigal works, and on a whim I’ve been trying to find them of late (really, really late; I did the google search fifteen minutes ago). So here’s as much of the text as I can remember:

Farewell my love, farewell my love and farewell again my heart … farewell my love.

[then it repeats or something … it’s been a while]

With thee alone my heart (either that or soul) can breathe. So take it now, I do entreat, in love complete.

Return at last, return, bring my heart again to me.

I may have left out a bunch, or I might have added something. But it’s one of the most beautiful songs I’ve ever heard/seen and I want it back, damnit:)

Anyone got a clue?
