unknown Scifi book

hi guys i have this book that i read some time ago like maybe in 2008-2010 and i loved it but i can’t remember the books name, i can remember pretty much everything in it but not the title.
Summary:
a boy about 16 named Fella is on a deserted part of the earth with other people. he lives in a orphanage there. everyone but him is made for harvesting. he and his best friend Gabby decide they have had enough of being told what to do and want to leave their “home” ( they do not know about the organ harvesting) they manage to escape via a underground train station thats under a lighthouse ( i think). they get to a new city, a place they were told was radioactive( story told by the people controlling them at their old home, to make sure no one wanted to try to visit the “radioactive” city). they have no idea whats going on but they try to fit in with their new surroundings. Fella starts realizing somethings up when he sees a protest poster of “Gabby”( his best friend) saying that she is sick and the “insurance” company( organ harvesters) is not giving her the medical treatment that she needs. fella finds out Gabby is being hunted by the guards from their old home in a attempt to catch her and get her organs for the sick"gabby". Fella decides that he needs to do something and he sets out to expose the company. There turns out to be a twist. Fellas dad is the owner of the company, he didnt know fella existed because the mother gave birth to him by the Orphanage before his dad had her killed in a car crash( i think). Gabby is being tracked by a chip inplant thats in her neck. i the end fella does expose his fathers company and almost dies doing it. also at the start of the book Fella and Gabby make friends with an old lady and her daughter who give them a room to use. they help them through out the story.

thats pretty much the main idea of what goes on in the book, this is probably a teen book seeing as i wasn’t into reading big as books at the time. iv been stuck on the name of this book for years and seeing as iv moved to a different city i cant go back to the library and check my reading records.
It would be amazing if someone could give me some suggestions.
