I’m **NOT **looking for a diagnosis but need to know who to go to.
Background.
I am an obese male. BMI is in the morbidly obese range. From time to time my left leg goes numb then gets pins and needles when standing.
I go monthly for a massage and when I’m lying on the table and my leg gets massaged in the upper thigh it is painful. Generally when sitting I have no issues with the leg.
I’ve lost about 70 lbs through WLS in the last two months and I haven’t noticed any change. I’m not sure if it’s a vascular issue or a nerve issue so I don’t know who to go to first.
Why not just go to a GP and have them refer you to the correct specialist?
From your brief description it could be almost anything from diabetes to gout. I really think you should start with a GP and have them do the diagnostics.
Visit your primary care physician/internist, who may be able to diagnose and treat you outright, but will otherwise be the person who can determine what kind of specialist you should be seeing, if any. It makes the most sense to bring new medical issues to the person who already knows your health history (assuming you have an established PCP) and is looking at your big picture.
(Critical care nurse/soon-to-be nurse practitioner, if you’re into credibility.)
Another vote for your Primary Care Physician first.
Most insurance these days requires a referral from your PCP before seeing a specialist.
You might also touch base with a WLS group. As you lose great amounts of weight, many things CHANGE inside. One change is physical, as your internal organs shift around without all the fat to hold everything in place. Another change is often hormonal, as hormones are fat soluble, and the depletion of the fat cells releases the hormones in great quantities. Women are usually the ones who experience tremendous hormone changes (for obvious reasons), but hormones affect men as well.
Weight loss affects balance and joint alignment, too.

Thanks everyone I will make a call to my Primary. I don’t think it’s a B-12 deficiency because my nutritionist would have said something when my last blood test came back.
