LNI = Late Night Imgur. Imgur is a popular image sharing website. Late Night Imgur is something of a tradition, a hold-over from when the userbase was a lot closer to being American-only. This isn’t as bad as it was.
Ogrish.com, Rotten.com, and Stileproject.com used to specialize in this stuff. Salon.com called Rotten.com “The Internet’s public enema No. 1” which might be over-stating the case a bit, but compared to, say, CNN or most medical textbooks, it was pretty out there. (On a quick glance: Still is.) Ogrish.com is gone and Stileproject.com is porn now, which I refuse to list here because kink-shaming is wrong. Ogrish.co is live, though, but it seems to be mostly ISIS execution videos now.
Speaking of, LiveLeak has a lot of gooey and otherwise gross videos, but not images, AFAIK. Don’t know why I mention it, really.
Then there’s the usual sites: Lemonparty, Meatspin, Goatse, Tubgirl, and the various BME (Body Modification Ezine) stuff, like “Pain Olympics”. Also, 1guy1jar (also “1 Man 1 Jar”) is pretty well-known, as is 1guy1screwdriver. If you can’t imagine what one guy and one jar and/or screwdriver would get up to which would earn them a place here, you’ll probably be happier not knowing. In general, stay away from googling 4chan or 8chan.

I have no idea if this website is still there after all these many years, but it’s a good story. My boss and I were going to a conference in South Carolina in 2002. We’d never been there before, either of us, so we thought we’d look up some things to do in the state. We were sitting at my cubicle, right out in plain sight of the department, and he suggested I type in ‘sightseeing in South Carolina’. The first result we got was called South Carolina Sights. We clicked on it and up popped one of those porn sites that you can’t close. Windows kept appearing all over the screen with some cheesy porn movie background music and women moaning. We couldn’t get the dang thing to close or the sound to mute! Finally, he reached under the desk and pulled the power plug! I still laugh about that to this day. We were so innocent about the internet back then.

My own (married) last name. I’m outing myself, if you are medically savvy, but there is a particularly gory and nasty disease of the genitals that was discovered by and named after one of my husband’s cousins x-times removed, a century or so ago. I don’t do any vanity searches, if you catch my drift. It’s doubly funny since my husband is an MD, and we’ve had to do searches in the past to see what his status is with Medicare, or his (former) clinic ratings, etc.
Not just cannibalism, but all sorts of sadistic sexual violence.
One time my coworker brought his 9-year-old son into work. The boy heard that I played ukulele, but wasn’t quite sure what a ukulele was, so I decided to show him by going to Google Images on a computer, and looking up “uke” - forgetting that “uke” is also a Japanese-derived term for the passive partner in a male/male relationship… Thankfully, I don’t think the relatively safe-for-work anime drawings that I got in the results were too bad for a 9-year-old to see. So that was more “something I wasn’t expecting” rather than “something terrifying.”
Could people who are contributing please explain what these things that must not be seen are, since they, ya know, must not be seen?

Teratoma is pretty easy, as there’s a Wikipedia page for it. It’s a particular kind of tumor with a defining feature that’s seemingly mundane, but the appearance can range from “huh” to “OMGWTFISIT.” The Wikipedia page has an innocuous picture and lots of text at the top, but if you scroll down you’ll see a couple of pics of more disturbing tumors.
The “terato-” prefix in the word means “monster,” as in teratology (the branch of medicine dealing with profound physiological abnormalities) or teratogenesis (the tendency of some chemicals to cause severe mutations and/or deformities).
Krokodil is a street drug that seems to have a strong propensity for causing necrosis. Pictures commonly show addicts suffering from massive gangrenous lesions, exposed bones, and that sort of thing.
“Lemon Party” refers to a picture of three elderly gentlemen engaged in a bit of bedsport, often linked to for shock value. Here’s the KnowYourMeme page explaining its background (that page, while NSFW, does not contain the image in question).

Lemon Party is just some old guys getting it on. It’s a slightly unexpected image, but it’s not goatse by any means.
NB: Google lies. Safe Search Off is still safe. If you want to intentionally find adult material, use Bing. I’m serious. They should work it into an ad, “Use Bing to earn thanks from your Thing!”
If it makes you feel better, Tubgirl is apparently “just” orange juice.
Oh, well as long as it’s just orange juice squirting out of her own ass onto her own face, I may as well use it as wallpaper for my desktop 
And to close out the descriptions, “blue waffle” is slang for a woman’s genitals that are puffy and bruised from overuse.
