vi or notepad, depending on which server I’m on.
I don’t like Notepad any more.
TextPad and Notepad are good. But DreamWeaver and GoLive sometimes have their uses, too.
I’ll usually do some rough layout in a WYSIWYG editor, and then switch over to raw code to tweak everything into place. But a point comes where you’re just changing the content, so I mostly just use Notepad and lots and lots of code reuse.
I was playing around last week, though, and the Mozilla Composer looks like it would work pretty well for my habits and purposes.

My favorite has always been Arachnophilia. It offers only as much assistance as you need, or you can simply use it like notepad.
