I am changing my name. Nothing exciting, just something I’ve been wanting to do for years and I finally have the money to do it. I was adopted at 4 and I’m going back to my birth name.
I’m trying to think of all the places I will see my new name…what am I missing? I have, so far:
Drivers license
Social Security card
Passport
phone, electric & water bills
email addresses
credit cards
checking accounts
checks
nameplate at work
name stamp at work
business cards
online registrations for websites
at the bottom of all my receipts
in people’s address books
in my own address book!
Student loans
Your resume
Medical records
If you have kids - their birth certificates
All your school records and diplomas (what do you do in this case to show these things are you?)
Do you need to register anything else? Firearms, dogs etc?
Subscriptions, magazine or other
Professional licenses (e.g., nursing)
Alumni associations, if you stay in touch
church/synagogue/mosque/temple registry
social organizations/clubs membership
This may vary by state. I am pretty sure that in Ohio, Birth Certificates do not get changed except through adoption.
One thing that tripped me up badly was when I had a “mailing address” for my driver’s licence and car registration. I had taken a PO bos for convenience and safety. But when I gave up the box there was no way to get it turned off as far as the DMV was concerned. They sent things to it for years, and refused or screwed up all attempts to go back to a single street address. This created backed up parking tickets and other official notices being declared delinquent because I never received them. When I went to court to clear things up they all had the dead PO Box number on them.
