I’m amazed that so many of the respondents seem to have skipped the part where the OP says “you can state what you want.”
I’d be willing to take it, if the questions made sense and the agent appeared to have her brain switched on.

Charlie don’t surf!
The people who sign up for something like that would have to be people who can easily adapt and generally more of the go with the flow and see what the day brings type, but it could be a blast for certain personality types.
I think that’s awesome.
Sure. I gave my wife a surprise honeymoon. She didn’t know our destination until the last flight leg. I had her put out clothes for several climates, then I packed her bag.
My mom’s AARP group does this all the time - everything from a day trip to a week. They’re told how long they’ll be gone and what to pack. Now, these are all bus trips, but it’s the same idea.
They’ve gone to historic places, crafty places, sometimes on boat rides, to all kinds of restaurants, and, of course, shopping. My mom tells me there’s a waiting list to get into thie particular AARP chapter because of these trips.
I think I’d be willing to give it a try if it wasn’t extremely expensive. I enjoy planning vacations, but I think it would be fun to be surprised.
It wouldn’t be for me even if they were a reputable company.
They could take me to what I call a once only location, you go there once; its interesting but once you’ve been there you’ve been there.
Or other quite reputable holiday themes that many people would totally love could leave me cold.
Wine tasting, fishing,Country and Western music festival, visits to a toy and or Creationist museum, lying on the beach for two weeks, learning how to be a chef or do arts and crafts…
All of these are legitimate holiday themes but would bore me stupid.
As others have said, I don’t think I’d find this appealing unless the questionnaire were so specific as to render the “surprise” part largely moot. There are already travel agents and tour companies offering packages like “California Wine Country” or “Historic France” and whatnot, so unless the mystery vacation package were a great deal I don’t see what would be the advantage over a conventional vacation package. Either way I’d be going someplace I’d never been before and having someone else handle the arrangements, and I don’t see any benefit to having no idea where I’m going to wind up. Someone who wanted a good deal and some spontaneity could just go to a site like Travelocity and choose one of their last minute fight/hotel packages.
Also, I like planning vacations. I like doing the research, and I like having the freedom to do what I like from one day to the next.
Now, for a short trip like a one day or weekend thing then I can see the appeal. I’ve heard that there are travel agencies in my area that arrange “mystery tour” bus outings like that. I live within day-tripper distance of a major metropolitan area, a national park and several other natural attractions, some significant historic sites, a couple of smaller cities and some cute small towns, so there are lots of interesting places such a tour could go. Since I’m not from this area it might be a fun way to see some places I wouldn’t otherwise have heard of. Even if we wound up somewhere uninteresting to me then I could probably manage to find something remotely entertaining about it, and I wouldn’t be out a lot of time or money if it turned out to be really boring.
I would take a surprise, (very) cut rate plane ticket anywhere in the world on short notice, but I’d prefer to find accommodations once I got there. And who pre-plans what they’re going to eat on vacation? A few posters up-thread wanted to approve the food–what kind of prison-stay vacations are you people taking?
