Yes, it is real. I have diagnosed it at autopsy before. It comes from the fact that a septum between the two halves normally disappears during embryologic life. Occasionally it persists, and often there will also be a divided uterus as well, in which case only sperm deposited in the vagina that leads to the side that has ovulated, will be responsible for pregnancy.

I believe there are reports of superfecundation (pregnancy by two fathers in both halves) in women with uterus didelphys, but I cannot recall. And I sure ain’t going looking for a cite. But if you google “didelphys” and hit “images”, you will be… well, depends if you’re like me, whether you’ll be fascinated, or grossed out.

Animal uteri are often divided by design, and so it is not a great jump to imagine that what we call a Mullerian duct anomaly in humans, is just a reversion towards functional design in other related mammalia. In other words, it ain’t a bug, it’s a feature.

It’s not rare (in fact, I have seen one, er, two)

My lady friend has a septate cervix. One uterus, one vagina, but the cervix has two separate holes. In general any doctor she’s asked has shrugged when she’s asked about possible consequences related to childbirth. Should she expect trouble when she has children?

Yes, there may be problems. She may have to deliver by Cesarian section. It depends how large the holes are.
