I’ve seen decent fake tits. It’s not that they can’t do good fake ones, it’s that, I suspect, a lot of the time the patient or surgeon has a mistaken idea of what makes breasts attractive.
I saw a show where they did a before and after of boob jobs and at one point a women took off her shirt to reveal asymmetric breasts: a large, but natural looking and attractive breast, and another that looked like she’d dropped her purse down her skin.
And, whadda you know, the surgeon pointed to the natural breast and remarked “oh dear, this one looks a little saggy, we’ll get it firmed up for you…”
Keep in mind, also… that the ultimate quality of breast implants is also related to how well the patient takes care of them in the first few months. There are special massages that are used to prevent scar tissue forming around the implant, and if the patient is not diligent about doing them, this can also cause the overly round & firm issues.
Also, safety issues aside, silicone implants usually look more natural and have a better “feel” than the saline implants that were so common for so long.
I had a coworker that absolutely adored the “obvious silicone” look on other women, even though she was quite well-endowed herself. She even went in for a consult with a plastic surgeon to see about having a breast reduction and THEN getting large silicone implants. Whatever floats yer boat, I guess.
