Authority for traffic lights & intersections?

I know everyone has encountered this situation. You’re at an intersection with a traffic light and notice there is a severe problem with the light timing, or which lanes are allowed to go where, or some other problem that is obvious - if only someone of authority would just come take a look so it could be fixed. How do you contact someone, or who do you contact?

I realize there different authorities for different roads (city, county, state), but assuming I know the difference, who’s in charge?
