As to your last question, why not try to avoid those that use any group of people as a frame of reference for contempt, when the attribute in play is both beyond that group’s control and something that is currently used to categorize said people as sub-human. If the word does none of those things in a different context–here’s the tricky part–then use it in that different, non-hateful context as much as you’d like.
Here’s an easy way to think of it. If you wouldn’t use the term in front of the people it uses as a handy little frame of reference and those who love them (let’s say if a mentally retarded child and her parents were in the room), then follow your instincts and don’t use it at all.
