Postal abbreviations are not supposed to be used except in addresses. It’s a style thing.

This depends entirely on what style you follow. Trying to make a general rule about it for anything larger than a large newspaper or publishing house is pointless.

Well, the Associated Press at least has a style guide that all contributing papers are required to follow. I spent a summer writing for The Battalion, the student paper at Texas A&M, and we were all required to follow the style guide. It’s a consistency thing.
