I’ve finished reading the latest edition, and the “one inch/glans-deep” advice is gone. The current advice is lots of lube and take it slow.
Though there are still the warnings about knocking against ovaries (and other internal organs).

No, but seriously, I dated a very crazy girl once and she wanted to do this thing. I would normally be into this kind of stuff but she has history with real rape and the whole thing turned me off completely, didn’t even follow through.
Didn’t matter because a few days later I broke up with her for unrelated reasons and a week later was served with a temporary restraining order. One of the reasons she gave was kidnapping (due to tying her up) and rape. She had bruises because she was anemic and other self-inflicted wounds but I was still in a world of pain. Luckily, because she had just spent 3 days in a loony bin for a suicide attempt, nothing came of it - but this still lead me to do a lot of research into this kind of stuff.
Basically, the general rule of thumb is, don’t do anything of the sort unless you’re like… a thousand percent certain your partner is not a psycho. If anything goes wrong, despite it being consensual, you’re still in a lot of trouble.
