TOS: I haven’t watched enough episodes recently, so I really couldn’t say.
TNG: The Inner Light, followed by the one where the Enterprise gets blown up over and over again (the title escapes me).
DS9: In the Pale Moonlight, the best ST episode I think I’ve ever seen.
VOY: I watch it, but I’ve missed TONS of episodes. I’m not even sure what my favourite one is so far.
Best episode definitely has to be “Loud as a Whisper” from the second season of TNG. It was the one that gave me hope for the series. First season was all a rehash of the original series. The idea of communication through a group that speaks for you was really original. And they get points for having an acotr play the role that really does have the disabilities portrayed by the character.
I just want to mention that a Star Destroyer could whup the Enterprise’s ass any day of the week.

Q Definitely the most interesting and the most entertaining.
The Borg Definitely the scariest. At least until the last couple of seasons of Voyager when they start to look like a cross between Abbot and Costello and a Veg-O-Matic.
Best Captain/Leader person: Jean-Luc Picard. I also like Sisko a lot. Although Kirk was a great character, I just can’t get around Shatner’s acting.
Best Second-in-Command: Spock.
Character Who Would’ve Been Least-missed if Killed: Wesley by a mile. Wesley and his phlegmatic whine needed to die.
Best Medical Officer: Bones. I like the EMH and Julian Basheer as well. Am I the only one who couldn’t stand Beverly Crusher? She so did not act like a scientist.
Best Sexpot: Female, has to be Kira Nerys. I don’t know why no one else is voting for her, I found her sexy as all-get-out, and I’m a straight female. If we’re talking about men, gotta vote for Benjamin Sisko. That gorgeous deep voice … mmm.
Best Ship Design: I think Voyager is a cute ship.
Best Engineer: I like O’Brien, but I don’t have really strong feelings about any of them.
Best Villain: Another vote for Khan. I always thought Q was stupid.
Best Episode: Unfortunately, I haven’t seen very many episodes of DS9, which is my favorite of the series.
So I’ll go with the one in TNG in which the ship is invaded by invisible aliens who, unbeknownst to the crew, start performing medical experiments on them. First they sever and then re-attach Riker’s hand, then they turn someone’s blood into “a liquid polymer” (I’m thinking rubber cement?) There’s a scene in which several characters go to the holodeck and try to re-create the experience. It’s really creepy and disturbing.
I also like the episode in which Riker is in a play about being in an insane asylum, and then it seems that he really is in an insane asylum. It doesn’t end very satisfactorily, but it’s suspenseful and good.
Best Captain – Catherine Janeway. I know I’ll be bashed for this, but I just like the way she handles herself. It also seems to me that she faces more adversary than the others, due to her being so distant from any support. Fire when ready.
Best Second in Command – Spock. His unemotional analysis of situations would help any Captain.
Worm Food – Gotta go with Wesley Crusher. Really annoying, and served no useful purpose.
Medical Officer – EMH. Really well acted.
Sexpot – Deanna Troi. Sexy, smart and well spoken.
Ship Design – Am I the only one who thinks Klingon warbirds are awesome to look at? If I have to pick a Federation ship, I’ll take the Enterprise D.
Chief Engineer – Scotty. If my ship’s falling apart around me, I’d want a Scotsman holding her together.
Best Villain – Khan. The ultimate psychotic villain.

Lizard, I have to question your assumptions. Not only was Worf not second-in-command of Deep Space Nine, I’m pretty sure he wasn’t even third-in-command (that would’ve been Jadzia).
Also, pigeonholing characters according to their place in the chain of command is pretty confining. I have to vote for Kira as best second-in-command, because her character was just the personality you’d want in that role: willful, opinionated, a born leader herself, but still willing and ready to defer to her commanding officer.
Of course Spock was a better character, but the way you’ve cast this poll I can’t indicate that properly. It makes it Spock vs. Kira, and that’s a false dichotomy.
All Voyager characters sift down to the bottom in all these categories, but that’s not really their fault; it’s because the show has so consistently been written by idiots.

TOS: The one with the tribbles. Almost too much fun.
TNG: “The Inner Light.” It’s the only episode of any Star Trek series that I actually bother to have on tape. Magnificent perfection, and it succeeded in giving me chills at the end when Picard gets the flute.
DS9: That episode where Sisko seems to be skipping through time, and his son is telling the story of this (as an old man) to a fan of his writings.
VOY: Hard to say. The series isn’t quite over yet, so I’m not sure if this category can be closed yet. But as of now, the best episode I’ve seen is the one with a member of the Q continuum petitioning to be permitted to end his life. Thought-provoking episode.

Worst Movie
Star Trek V: The Final Frontier. May this abomination be stricken from the memories of all who saw it. May Shatner never be permitted to write or direct again. Not only was it written and directed poorly, it actually looked poor. The visuals and special effects were stunningly sub-par, even for the time.
Worst Episode
TOS: There were some silly ones, but I can’t think of any sillier than “Spock’s Brain.”
TNG: Also had some very silly episodes, but the one that leaps to mind first is an early first or second season episode, “Shades of Grey” involved Riker’s brain being tapped to stimulate memories. Basically a flashback show…pointless waste of time.
DS9: Can’t think of any off the top of my head. I’m actually surprised at this.
VOY: The one with Janeway and Paris turning into what looked like mutant salamanders. Good God, what were they thinking?
