It may very well be a pre-erection migraine, however, ANY “first, worst headache” should be evaluated by a neurologist or neurosurgeon. It could be an arteriovenous malformation or a Berry aneurysm. Both can be fatal without treatment. If you are becoming hypertensive (meaning your blood pressure goes up) during arousal, the AVM or aneurysum will spasm, causing that first, worst headache. That could happen a number of times but eventually, the area will give way and bleed. Once that happens, the chance for recovery approches zero.
Go to a doctor. Spend the money. Its the only life you have.
