What would his lackeys be sick of? They probably have the nicest lives of anyone in North Korea. That’s a strong incentive not to oppose Kim or stage a coup- and of course if they did try to get rid of him they could end up dead and with their families dead or in a concentration camp. Those are some very powerful incentives. And yes, I think in situations like this, a sociopath leader will surround himself with people who are pliable or just as crazy and give them free reign to go nuts on his enemies.
