Major league ballplayer killed by fire extinguisher

I was reading “The New Bill James Historical Baseball Abstract” (an excellent book if you like baseball and statistics). For each decade, James mentions some ballplayers who died and the cause of death. Most are pretty ordinary. But one said,

“Len Koencke, fire extinguisher”

No further explanation was given. What’s the story of Len and the fire extinguisher?
