For all you Lost fans, I just realized that the actor playing Jacob is the same actor who played the German clock specialist in Nothings Perfect.
I’m fairly sure that’s the season 5 premiere “Three Doctors.”
I got to see part of the filming of that episode! A friend and I were in the area in July 1993 and decided to check out Roslyn, not knowing they were shooting that day. From across the street we saw a stunt driver driving Joel’s truck off the road and Janine Turner running after it and yelling “Fleischman!” They did 2 takes. Then we saw most of the cast (many in surgical garb for the dream sequence) gather outside Joel’s office and shoot the breeze for a few minutes before going inside for interior shooting. That was about it, but it was pretty cool.

Oh, in re: Northern Exposure — in every one of my pregnancies, at some time or another, I’ve imitated Shelly who, in the early part or her pregnancy, complains about her " 'roids," and eases herself down in a chair as though she were 9 and a half months pregnant. When really, she’s really only about nine and a half minutes pregnant! 
That was always my favorite one, too.
