I’m not a doctor either, but AIUI, a buccal swab allows you to determine the subject’s sex chromosomes. That doesn’t necessarily tell you what their genital configuration or hormone levels (e.g., testosterone) are like. For example:
Transgender women who’ve had top and bottom surgery and hormone therapy may be physically indistinguishable on casual examination (and in sports competition) from cisgender women, but most of them have XY chromosomes.
And so on. It is really not so simple as “female = female genitalia = XX chromosomes = female gender identity = female-norm levels of sex hormones, so if we test any one of those then we’ve answered the question.” Aspects of sex and gender identity are complicated and not always consistent with a simple binary model of sex and gender.

This is not necessarily true. There are female genital mutilation laws on the books of various states which may come into play when considering bottom surgery for minors. In Florida, for example, it is a first degree felony to excise any part of a minor’s labia or clitoris, regardless of the patient or their parent’s consent (it is a third-degree felony for the parent to provide consent); there is an exception for licensed healthcare professionals but only if “necessary to preserve the physical health of a female person”, emphasis mine. Fla. Stat. 794.08.

I recently heard that they banned chromosome testing for the Olympics in 1996 because when they did the testing, 8 of the 3,000 failed - 7 due to androgen insensitivity, and 1 for an intersex condition. Interestingly, 7 out of 3,000 is a lot more than 1 in 15,000 - it could be that androgen insensitivity helps athletic performance, but I wonder if androgen insensitivity is just a lot more common than we think. My understanding is that it basically goes undetected unless the women tries to have a child and has enough resources to get tests to figure out why she isn’t getting pregnant*.
