Yup, that was the point of the song. As it starts, you think it’s about when his wife/girlfriend (or whatever) “ran away.” Notice “ran away”? That’s an unusual way to speak of someone leaving you or breaking up with you. But the big reveal is later:
The song is about descending into madness. Was it his dog or his lover that left him? Can he tell the difference any more?
You know, I always thought that was just invective, not literal.
