Having the tongue hang out the side of the mouth makes him look like he has an upset tummy. I think the central tongue position is best.

Well, according to the Visibone Color blind color chart, most color blind people only see shades of yellow-orange and faded blue (from bright and intense to dark and washed out).

And so, you can use those two colors which will mean that (almost) everyone sees them exactly the same, i.e., yellow-orange and faded blue. You can experiment to see which looks best as face v. tongue. Though, I think a light yellow-orange face will show off the black lines and dark faded blue tongue best.

Or, you can use colors which to the color sighted look like an olive face with hot purple/fuscia tongue, but to the color blind will look like yellow-orange and faded blue.

Either way, as I’ve mentioned before, whatever colors you use, there needs to be high contrast. Don’t put dark on dark or light on light.

I think the ":-P"smiley from Yahoo IM is just about perfect. If you added the eyes from your example #2, I think it’d be on the spot. Not that my opinion is worth much.
