So Michael Vaughn’s real name isn’t Michael Vaughn.
It’s been firmly established that Irina did indeed kill his father; she knows who he is, she remarked one time that he looks just like his father, and they just discussed it in the last episode. And I’m almost positive that Michael’s father has been referred to by name at some point; I want to say he’s on the Wall of the Fallen or whatever it’s called in CIA headquarters? So if they’re both Vaughns, they must have changed their name from something else when Michael was very young.
My guess: the father was an Alliance agent but switched sides to the CIA, who gave him and his son new identities and used his inside knowledge in their war with the Alliance. So when Sydney turned against the Alliance, the CIA had Vaughn be her first point of contact. Now, Michael is either not going to survive this accident, or will be in a coma, so Sydney will have to find all this out on her own.
Somehow this must tie in with the fact that the senior Vaughn delivered the infant Nadia to Elena’s orphanage. He must have known that Elena was Irina’s sister and would see to it that Nadia be trained as an agent. So the father was a double agent and Rambaldi disciple, working for Elena to help fulfill the prophecy. Irina, in the course of trying to stop her sister, killed daddy Vaughn.
Lauren was also working for Elena, so that’s how she knew about the plot to trick Jack into killing the fake Irina so they could keep the real one prisoner. Lauren also knew of Nadia’s existence and that she was an agent; hence her revelation to Sydney before she died that it wasn’t a coincidence that the two sisters were agents.
(That last two paragraphs just came to me as I was typing them… 
)
