I need the below words translated from English to Spanish.
In short, I am developing some “email forms” that will be presented and filled out on my company’s web site. I don’t exactly trust online translator sites, but they may suffice for my purposes. Just thought I would check with you guys first.

You could always go for the bulky “correo electrónico” for e-mail if you want to be really Spanishy. But I’d just put “e-mail”, myself. They’ll know what it is.

One suggested correction:
Make it “Modo preferente de contacto”.
Otherwise, it could be taken to mean “Style of the preferred contact (person)”.
If I may, translation’s good but why would you ask for a middle name as opposed to a full name? After all most people don’t go by their middle names thus hardly anyone mentions them that way, so why not ask for a full name? As in “Nombre y Apellidos”
We are asking for First Name, Middle Name, and Last Name. I already had the translation for First and Last.

I am using clear in the sense that users will be clearing any entered data on a form.

Add a “Nombre de” before Cooperativa.
Is the search to look up various Credit unions?
You could use Borrar for Clear, then (at least, I’ve seen some pages using that)

I’ve seen all those variants used in electronic forms in various Spanish websites (including the online dictionary!).
