I do know that it’s not a canned theme, they record it fresh every time they go into the scoring session.

Maybe this doesn’t qualify because it was in a movie as opposed to a tv show, but there was that one scene in Spiderman where a street musician can be heard performing the 1960’s Spiderman cartoon theme song
This one is a bit of a reach, but bear with me -
In Season 3 and 4 of Doctor Who, we found that the Doctor’s nemesis, the Master, had a drum beat embedded in his skull, helping to drive him insane and infecting those under his mental control. It was a simple four-beat rhythm, dum-dum-dum-dum, dum-dum-dum-dum, like this.
It was also the exact same rhythm, slowed down a bit, of the show’s iconic main title theme. The implications are… interesting.
I’m guessing the cast of Two and a Half Men lipsyncing the theme (“Men, men, men, men, manly men, men, men…”) doesn’t count.
Vonda Shepard turned up on Ally McBeal multiple times to perform but I don’t recall her playing the theme song itself diegetically.
