Mick Jagger is really ugly, so he gets my vote. Jimi wasn’t pretty, but an average guy doesn’t belong on the same list as Mick.
I can’t believe I’m the first person to mention Bob Dylan.
Does anyone else remember the Bloom County strip that pointed out the gorgeous girlfriends and wives of rock stars like Ric Ocasek (married to Paulina Porizkova for more than twenty years) and Billy Joel (formerly married to Christine Brinkley)?

That is somewhat debatable, but he certainly qualifies as a Rock Star. You need only search on the phrase ‘Bob Dylan Rock Star’ to find ample evidence that it is so. It’s not exactly a discriminating title. If you check the lyrics of *Rock and Roll Heaven you’ll see that Bobby Darin qualifies for entry there, certainly Dylan will be admitted as well. He has been inducted in the Rock and Roll Hall of Fame, and Like a Rolling Stone *was #4 on the VH1 list of 100 Great Rock Songs. So I think he strongly qualifies as a Rock Star.
The man could have been in the classic Twilight Zone episode “Eye of the Beholder” and wouldn’t have needed makeup.

Now photos are such easy pickings. Great choice, but the then photo suffices.
Wow… I’m sort of amazed that I’m saying this, but I don’t know if this new Shane is doing much for me. He looks… ordinary. Pretty normal. Which is not what I’m used to seeing when I think MacGowan.
That Spirit dude is out there. Most of these guys, like Ocasek, are “interesting looking.” Not classically beautiful, but you can see how someone can be drawn to their “striking” looks. Interesting looking guys fronting bands seems to work. The real measure of ugliness is if a guy is so ugly, he actually turned people off the band.
These suggestions are extremely bad. Jimi Hendrix is just straight up handsome. Mick Jagger was also rather handsome when he was young, and still probably looks better than the average 68 year old. Keith Richards looked fine when he was young too.
My suggestions would be Meatloaf and Lemmy (wtf is up with those moles)
But really, maybe half the rock stars that comes to mind are less good looking than Jagger (when he was young) and Hendrix.

Stevie Ray Vaughn was pretty damn ugly. But he played so good your eyes would water, so you couldn’t get a good look at him.
The moles are filled with alcohol & nicotine.
But nobody VOTED for him. He was mentioned in the OP, but nobody’s picked him. The guy is barely human.
How Geddy Lee hasn’t been mentioned is beyond me.
Same with Keith Richards back in the day. He’s worn out looking now but was gorgeous.
