What is this document and how do I get money from it

Hey there, I have this document entitled “Sworn Statement in Proof of Loss” and I think I can get $189.10 from it, but I’m not sure how. Please help me.

What happened was, a few months back, I retrieved my stuff from a Public Storage locker and discovered that water damage had left nasty marks on my mattress. I contacted the insurance company through Public Storage and they assessed the damage and sent me this document. It has no address on it to where I should send it, and I’m not quite sure how to turn it into money. Towards the bottom it says “The furnishing of this blank or the preparation of proofs by a representative of the above insurance company is not a waiver of any of its rights,” and then it says “Please notarize.” My dad’s friend notarized it (she’s certified in Ohio, but I live in Chicago) back in December and I still haven’t done anything with it.

Does anyone know what I should do with this? Is the notarization still valid or whatever?

I suggest contacting them again and asking them.

With the notorization it is your affadavit of loss. Take it back to the storage company if you don’t know what else to do with it, but have a copy made for your records.
