I’m not sure that even if the torpedo had been fired it would have inevitably led to WW3. Tensions were high, and so it may have, but I think that Kennedy wouldn’t have gone full on MAD based on a single attack on a military ship by a beleaguered sub that may not have been given the command to fire from on high. This wouldn’t really make sense as the first step in a full out first strike that would demand a response. Chances are Khrushchev would have immediately contacted Washington to make it clear that he didn’t give the order and to would back down immediately less they get Washington got the wrong impression.
